QUALIFICATIONS:
  1. Education: Minimum Master’s Degree in Social Work or related field required.
  2. Licensure: Valid Ohio LSW or LPC required.

POSITION SUMMARY:
The Case Manager is responsible for supporting Bellefaire JCB’s Jewish Youth Mentoring Program. The Case Manager maintains contact with their caseload and provides monthly support. Evenings and weekend availability will be expected to maintain contact with mentorship matches and support program events.

RESPONSIBILITIES INCLUDE:
  1. Provide supportive supervision to mentors, mentees, and guardians, as appropriate, to maintain the integrity of mentoring matches (community based) through the entire match life cycle (i.e., the interview, training, introduction, supervision, assessment and closure).
  2. Collaborate with colleagues and the Jewish Youth Mentoring Program to ensure timeliness of program and match support the inception of new matches.    
  3. Support and attend program events including group outings, program fundraisers, volunteer appreciation meetings, etc.
  4. Remain in compliance with Jewish Youth Mentoring program standards, including specified documents, policies, practices, procedures and materials.
  5. Responsible for all administrative tasks including, but not limited to, record keeping, supervision, attending staff meetings, monthly individual meetings with mentors, guardians and mentees, and other meetings and functions of agency, overseeing volunteer committees, attendance at program/ agency events and outings.
  6. Serve as a resource, referral source, and advocate for Bellefaire JCB to appropriate community services.
  7. Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and program standards.
OTHER DUTIES:
  1. Assist with and attend special events and annual meetings as required.
  2. All required trainings, certifications and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards.
  3. Respect the privacy of mentees and clients and hold in confidence all information obtained during the client’s treatment.  All client-related documents should be handled in accordance with Agency guidelines on confidential material.
  4. Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures.
  5. Other duties as assigned by management.
